R.J. MCELROY TRUST GRANT APPLICATION AND GUIDELINES

Application Guidelines:

Before you begin the application, please talk to the Executive Director, Stacy Van Gorp.  Call 319-287-9102 or e-mail vangorp@mcelroytrust.org.  We ask you to do this so we can learn more about the project and to ensure eligibility.

A.  Describe the community/need or problem being addressed by this project.

B.  Project Description.

  1. Please provide a summary of the project.  50 word limit.
  2. Describe the specific activities planned and the timeline.
  3. Describe expected outcomes. Be specific.  How will you know if the project has been successful? (If awarded funding you will be asked to report on these outcomes at the conclusion of the grant.)

      A.  How will the funds you are requesting be used? 

      B.  Other Funding Requests. Please list other sources you are requesting funding from, the amount you are requesting and if funding has been approved. Please include

              general fundraising activity and in-kind services/gifts.

      C.  Plans for On-Going Funding.
